Sarah Jane Rowland

Sarah Jane is Project Lead at The Occupational Therapy Service, having previously worked extensively in the transformation of Occupational Therapy and Reablement services. Sarah Jane has a wealth of experience and an analytical eye to explore where service improvement and efficiencies to be implemented to maximise service delivery. She has been instrumental in supporting partnership working between Health and Social care to maximise patient flow from the acute setting into the community. Understanding the value of the Neighbourhood Model and the significance of prevention has led to positive outcomes. Sarah Jane is passionate about Occupational therapy and will ensure that it is at the centre of her practice when implementing change with organisations. Most recently she has led a proportionate care project to undertake Therapy led reviews ensuring that the right care is delivered to the person, and is currently leading on a large scale project to reintegrate health OT and reablemement
